Your time is valuable, especially if you are a busy professional.  We travel to your home apartment, your office workplace, (or, your hotel room if you are currently visiting New York City).

Even if you are retired or semi-retired, you are still a busy New Yorker.  And, depending on your schedule and your preferences, you may prefer to have a Doctor of Physical Therapy travel to you if or for you to sort out your ache, pain, or balance/movement issue.

Additionally, many clients will offer to show us their bed pillow and/or ask us to evaluate their mattress.  We will check the amount of support and cushion that your bed pillows and mattress are currently providing for you.  We may make specific suggestions on types of pillows or mattress based on your condition and depending on your current needs. 

Plus, for our client's who are 65+ or 75+ who have fallen once in the last 1-2 years, we will also offer a home assessment.  During this home assessment, we will evaluate the safety of your home apartment.  Often times we will be communicating these findings with you (the client) directly as well as your family members (spouse, son, daughter, brother, sister, etc).  We understand that it's important to you maintain your dignity and integrity while thriving at home and living your life independently.  Also, we know it's important to you be able to maintain control on your life's decisions.  We take that very seriously and will respect your ultimate decision on any potential safety suggestions in regards to your home. 


5.) Schedule Dates and Times That You Prefer

Depending on the current schedule of your doctor of physical therapy, you have the ability to schedule your home visits at your leisure.  Please do let us know your time and day preferences.  Some of our clients prefer mornings or mid-day, while others prefer afternoons or evenings.  At the same time, some clients want Monday, Wednesday, Friday visits and some like Tuesday/Thursday visits.  If a new client is in a lot of pain or they require a lot of one-one-one time and attention, then often times they request 5 to 7 days per week of concierge physical therapy treatment, and then wean down from there in the future.

6.) Same Therapist (Doctor of Physical Therapy) Visiting You for Each Home Visit

At some outpatient rehabilitation offices, you may be seen and treated by several different therapists and providers.  However, at Concierge Pain Relief, you will be evaluated and treated by the same doctor of physical therapy for all of your follow up visits.  This is assuming that you really like your therapist.  If there's any issues or if it's just not the right fit, then let us know, and we can arrange another doctor of physical therapy for you.

SI Joint Pain Dysfunction Self Test

SI Joint Dysfunction

The Sacroiliac Joint (SI Joint) is involved in about 22% of patients with lower back pain.

The pain and dysfunction of the SI Joint is often overlooked and underdiagnosed by physicians.

The pain pattern can overlap with spine (lower back) and hip conditions.  Which is the main reason why it is often missed by physicians or physical therapists who are making a diagnosis.

Some physicians may not think of the SI Joint as a pain generator in the case of lower back pain.

It's possible that some doctors may not know about SI Joint dysfunction because they do not se it enough or treat it enough.

Patients may end up seeing multiple physicians, doctors of physical therapy or other health care providers, get multiple diagnoses, and get many imaging studies including x-ray, MRI, or diagnostic ultrasound imaging. 

The only way that true SI Joint pain can be diagnosed is by a proper movement screen typically performed by a doctor of physical therapy...

Where you (the patient) may have pain with bending, twisting, prolonged sitting, getting up from a chair, or walking up stairs. 

SI Joint Dysfunction - Self Test: "Fortin Finger Test"

The Fortin Finger Test is a diagnostic tool to help find and confirm SI Joint dysfunction. Here's how the test is performed:

The patient will point to the spot on their body that is most irritable or where they feel a majority of their current lower back pain.


Once the individual points to the location, the doctor of physical therapy memorizes the spot and allows the patient to relax. 


Five to ten minutes later, the doctor asks the patient again to point to the area where they feel the most pain. 
A positive Fortin finger test is determined by when the patient twice identifies their most painful region within one centimeter of the area known as the posterior superior iliac spine.

The test is positive (+) because it shows the patient is able to consistently identify the source of pain even though some time has passed.

A positive result from the Fortin Finger Test indicates a strong likelihood that the patient is dealing with an SI Joint condition.
